Dengue Fever in Koh Samui: Symptoms, When to See a Doctor & Treatment

Dengue fever is a real risk in Koh Samui and across Thailand, particularly during the rainy season from May to November. Every year, tourists and expats are affected — and while most cases are manageable, dengue can become life-threatening without proper medical supervision. Here is what you need to know.

What is Dengue Fever?

Dengue is a viral infection spread by the Aedes mosquito, which bites mainly during daylight hours — especially early morning and late afternoon. There are four dengue virus serotypes (DENV 1–4). A previous dengue infection does not protect you from a different serotype and can actually increase the risk of severe disease on a second infection.

Dengue Symptoms

Symptoms usually appear 4–10 days after a mosquito bite and include:

  • Sudden high fever (39–40°C)

  • Severe headache, especially behind the eyes

  • Muscle and joint pain (sometimes called 'breakbone fever')

  • Nausea and vomiting

  • Skin rash (usually appears 2–5 days after fever onset)

  • Fatigue and exhaustion

Dengue often looks like a severe flu. Many tourists assume it is food poisoning or a bad cold — this is dangerous. If your fever came on suddenly and is accompanied by severe body aches, get tested.

Warning Signs of Severe Dengue

Some people develop severe dengue (previously called dengue haemorrhagic fever), which is a medical emergency. Seek urgent care if you notice:

  • Severe abdominal pain

  • Persistent vomiting (3 or more times in 24 hours)

  • Bleeding from the gums, nose, or in vomit or stool

  • Blood under the skin — looks like bruising or tiny red spots (petechiae)

  • Rapid breathing

  • Fatigue, restlessness, or confusion

  • Sudden drop in fever accompanied by feeling very unwell

Treatment for Dengue

There is no specific antiviral medication for dengue. Treatment is supportive:

  • Rest and avoiding physical exertion

  • IV fluid replacement if dehydration is present or severe

  • Paracetamol for fever and pain — do NOT take ibuprofen, aspirin, or naproxen as these increase bleeding risk

  • Regular monitoring of platelet counts and haematocrit

  • Hospital admission for severe dengue cases

How to Protect Yourself from Dengue in Koh Samui

  • Apply DEET-containing insect repellent throughout the day

  • Wear long-sleeved clothing, especially at dawn and dusk

  • Sleep under a mosquito net or in an air-conditioned room

  • Eliminate standing water around your accommodation

The dengue vaccine (Dengvaxia) is available but is only recommended for people who have previously had dengue fever. Ask our doctors for specific advice.

How do I know if I have dengue or just a bad cold?

Dengue typically causes a very sudden, high fever with intense body aches and pain behind the eyes. A cold rarely causes fever above 38°C or such severe muscle pain. If in doubt, get a dengue test — it takes 30 minutes and is affordable.

Is dengue fatal?

Most dengue cases are not fatal when managed properly. Severe dengue can be life-threatening, particularly in people with a previous dengue infection. Early diagnosis and monitoring is key.

Can I fly home with dengue fever?

We recommend you do not fly while acutely ill with dengue. The change in altitude can worsen symptoms. Consult our doctors before making any travel decisions.

Is there a dengue vaccine?
