Dog Bite, Pig Bite or Monkey Scratch in Koh Samui? Rabies Vaccine — What to Do Immediately


Koh Samui is home to large populations of stray dogs, macaque monkeys, and nearby islands such as Koh Madsum (Pig Island) where free-roaming pigs interact closely with tourists. All of these animals can transmit Rabies.

If you have been bitten, scratched, or licked on broken skin by any animal in Koh Samui or surrounding islands, you need to take action immediately.Rabies is almost always fatal once symptoms appear — but it is completely preventable with prompt treatment.

⚠️ Is Rabies Present in Koh Samui?

Yes. Thailand is classified as a rabies-endemic country. Koh Samui has had confirmed rabies cases in animals.

Any mammal can carry rabies — most commonly dogs, cats, monkeys, and potentially pigs. The risk is real and must be taken seriously.


🚑 What to Do Immediately After a Bite or Scratch

Act within minutes. The following steps are critical:

Step 1: Wash the wound immediately with soap and water for a minimum of 15 minutes.This is one of the most effective ways to reduce rabies risk.

Step 2: Apply an antiseptic such as povidone-iodine or alcohol if available.

Step 3: Do NOT close the wound with bandages — leave it open initially.

Step 4: Go to a clinic or hospital immediately, even if the wound seems minor.

💉 Rabies Post-Exposure Prophylaxis (PEP)

Rabies PEP (not the same as HIV PEP) is a course of treatment that prevents rabies after exposure. It consists of:

  • Rabies immunoglobulin (RIG) — injected around the wound on day 0 for high-risk exposures

  • Rabies vaccine injections — given on days 0, 3, 7, and 14

Treatment must be started as soon as possible, ideally within 24 hours. Do not wait.

❓ Do I Need Treatment If the Animal Looked Healthy?

Yes.

Animals can carry rabies and appear completely normal for weeks. You cannot tell by appearance whether an animal is infected.

All bites and scratches from dogs, cats, monkeys, and pigs in Thailand should be assessed by a doctor.

Monkeys at popular tourist locations are a common cause of bites and scratches.

They can carry:

  • Rabies

  • Herpes B virus (rare but potentially fatal)

Any monkey bite or scratch should always be medically assessed.

🐖 Pig Bites at Koh Madsum (Pig Island)

At Koh Madsum, pigs roam freely and often approach tourists for food. While generally friendly, bites and scratches can occur — especially if animals feel threatened or are being fed.

Important risks from pig bites include:

  • Potential rabies exposure (any mammal can carry rabies)

  • Bacterial infections from the pig’s mouth and environment

Even small pig bites or scratches should be:

  • Washed immediately

  • Assessed by a doctor

  • Considered for rabies vaccination depending on the exposure

Do not underestimate pig bites — they can easily become infected.


🛡️ Pre-Exposure Rabies Vaccination

If you plan to spend significant time in Thailand — especially if:

  • Working with animals

  • Visiting islands like Koh Madsum

  • Travelling to remote areas

  • Cycling or motorbiking

A pre-exposure rabies vaccine (3 doses over 21 days) is strongly recommended.
